All Taxi Drivers must have a Medical when applying for first Badge or Renewal under any local authority.

Please bring to your D4 and PCO medical:

  • D4 Medical Form

  • Photo ID

  • Driving glasses/contact lenses & prescription

  • Details of medicines

  • Medical consists of:

  • An eyesight test using a Snellen’s chart

  • Blood pressure checks

  • Medical history
